At Sonne Health, our mission is simple: to help build healthier rural communities by providing accessible, patient-centered primary and preventive healthcare, right in your home. Founded and led by a Preventive Medicine and Family Medicine physician with over 15 years of experience in rural healthcare, we understand the unique challenges folks in small towns face accessing quality care.
From providing direct patient care in rural federally qualified health clinics and critical access hospitals, to leading regional public health initiatives, we bring expertise, compassion, and practical solutions to support your health and well-being, wherever you live.

How It Works
Primary and preventive medical care services to folks in Central OR.
Making house calls, coming to your home to see you in-person for your initial appointment, with a combination of virtual, telephone, and in-person visits thereafter.
We don’t bill insurance: the cost is out-of-pocket to you, no middle-organizations.
You can enroll for 3 months at a time, for $300, and will be billed $100/month for those 3 months.
Each additional household or facility member is an additional 25% off.
We will try to respond to urgent concerns within 3 days, and non-urgent concerns within a week. For anything more acute or concerning that can’t wait the 3 days, or if you haven’t heard from us, you should seek more immediate care, e.g., an emergency room or urgent care.
